So they dubbed it and never released it? Why is that?
The story is as it follows:
After the S DVD release flopped back in 2011, a Brazliian fanclub that exists since the late 90s and had ties with Toei backed the release of new content.
They actually paid (!!!) for a dub of the R movie. They scripted the dub and selected the voice cast. Moon, Mars, Jupiter, Tuxedo Mask, Luna and Artemis were voiced by the same actors who voiced them in the Classic dub. Mercury and Venus's VAs from the R-Stars dub were brought back, as well as Chibiusa's only voice actress to date.
However, even though Toei didn't have any expenses with the dub itself, they gave up on releasing the movie on home video in the last minute, afraid of low sales. They didn't even get the rights to the dub in the end.
The 90s movies are coming to Netflix Latin America the next month, and Brazilian Portuguese dubs are included. The S and SuperS movies probably got a dub using the cast from Eternal (which completely ignored the Classic dub cast, and therefore most of the actors involved in this unreleased dub), but we are yet to discover whether Toei tried to rescue this R movie dub with the fans or comissioned a new one.

Good luck on trying to do that. I just hope it CAN be saved. We already lost the original masters of the DIC Dub and it'd be a shame if the Optimum Dub suffered the same fate
Even if the DiC masters were technically lost, it might not really be a huge deal. The dub was highly likely produced completely in 480i, so I'm willing to bet the footage on the DVDs isn't that far off in quality from the official masters. Assuming some company is ever able to rerelease it they could probably rip it off the DVDs and touch it up a bit. Even then, you know never know, the masters might pop up one day. Many believed the DiC soundtrack was lost forever, and just last year most of the soundtrack finally got leaked online.

Of course this is all hypothetical, it's unlikely the old dub will ever get a rerelease anyway.

Is the version of Moonlight Densetsu sung by the Crystal cast from the 20th anniversary CD considered lost media? I can’t find any uploads of it on YouTube or anywhere.

Is the version of Moonlight Densetsu sung by the Crystal cast from the 20th anniversary CD considered lost media? I can’t find any uploads of it on YouTube or anywhere.
Is that CD unavailable in the (primary or secondary) market now? If not, it's not lost media. :)

I’m not sure to be honest, but I’d say that that version of the song is lost considering that I can’t find it anywhere to even listen to it.

can we consider the hero club videos lost media until they get uploaded?
(if the textless version of the first Sailor Stars opening was in one of them, surely textless versions of the other versions of the various openings are in them, too.)
I know. It's hard to find the textless versions of that opening alongside the first Sailor Moon Classic opening (with the 3 Senshi silhouetted from the veils all the way), Sailor Moon S's first opening with just the 5 Senshi, and Heart Moving's first ending version (there are 2 versions of the Heart Moving end credits sequence). Let alone the TV size versions of Kaze mo Sora mo Kitto (plus the full slow first version of the song) in both forms from a CD album quality.

I hope that someday we get to see the pilot of the hypothetical Codename Sailor V OVA if there is anything. We have seen some of the concept art.
