In a close up shot of Usagi, the shot is frozen as her mouth stays closed and does not move when she says the line “About her breaking the toy,” This error is not present on the Pioneer DVD version, as her mouth does move in that version. It’s especially jarring since this isn’t some far away shot but a close up of her face.

After that, while Makoto's trying to fix the spatula, the Hulu subs has Rei say "She's pretty angry, all right." The home video subs changed it to "She's pretty pissed, all right."

When “Route Venus” starts playing, only the first line’s lyrics are subbed (as “Don’t forget about me…”). The rest of the song’s lyrics are not subbed.

So I’m guessing that there’s a “don’t” missing between the second occurrences of the words “you” and “even”. Kyusuke’s later insistence in these subs that he doesn’t need to practice supports this.

Chibiusa’s use of “minna-sama” is subbed as “everyone”.

And, right out of the gate, the subs jump into making pun that doesn’t exist in the dialogue. When Luna describes Chibiusa’s portrait of Usagi as “Kanpeki” (meaning “perfection”), the subs translated it as “Purrfect!” Ya know, cuz Luna’s a cat and all? Ugh.
Changed the line to "Nice, Chibi-Usa. Looks exactly like her." This removes the cat pun, Even the dub is close to what the sub said.

Consistent with episodes of previous seasons, “Nikuman” is subbed as “pork buns”.

Viz’s subs title the episode as “Don't Lose Sight of Your Dreams: the Mirror of Truth”

In the post-title card scene, the subs have Chibiusa say “I treated you to a pork bun, so we had a deal!” The problem here is that we can see that she treated Usagi to a bunch of nikuman rather than just one, so “a pork bun” ought to be “pork buns”. And the subs did say “pork buns” back in the pre-title card scene, so the subs in this scene ought to be consistent with those in that scene as well.
Changed to "Look, I already kept my end of the bargain by treating you to some pork buns, didn't I?!"

Usagi’s response to Chibiusa saying the above line about their deal is subbed as “It’s up to me what I do with it!” Do with what, exactly? I don’t understand what this line is supposed to mean, as the scene depicts Usagi about to punch Chibiusa in the head.

“Ekaki-san da,” is subbed as “An artist.”

There is a slight animation error when Jun-Jun speaks her line of “Why do we get iced out? It’s not fair.” When she speaks this line, her mouth stays closed and does not move until about one-third of the way through her speaking this line. In the Pioneer version, her mouth opens and moves almost immediately when her dialogue starts up. Here, though, it stays closed for the first-third of her line of dialogue.
Fixed the glitch in the new upscaled masters, but this line is changed to "Why are they shutting us out? It's not fair."

The subs have Jupiter’s reflection talk about how Makoto’s desire to be more like a “young girl” instead of as more like a “girl”. The repetition of the word “young” in the subs makes it sound like Makoto’s trying to hold on to her youth, when the word being used in the dialogue is actually “onnanokorashi”, which means “feminine” or “girly”. The visual support this in their depiction of Makoto being tempted with a beautiful (to her) womanly dress. So it would make more sense for the uses of “young girl” in the subs to be replaced with just “girl”.

Usagi’s line that was subbed in episode 162 as “Your pain is my pain.” is subbed the same here.
